To register you or your child for Champion Studio Classes complete and return signed Registration Form, Policy Sheet and Medical Release Form.

Attached is a list of required dance wear and a coupon which may be used at Distractions, located at the front of the studio. Distractions will have all the required apparel you will need. For any questions on classes or registration please feel free to call Champion Studio at 593-2729.

Champion Studio Policies

Welcome to Champion Studio! Our programs and professional staff combine to offer your child a positive and enjoyable learning experience. This information will help acquaint you with our policies and procedures. Please keep this information sheet for policy reference.
Registration Fees: There is a $35.00 registration fee for each student. This fee is non-transferable and non-refundable. An additional
$15.00 service fee is required for each time a student drops and later re-enrolls.
Tuition: Tuition is due and payable the first of each month, and all monthly payments are non-transferable and non-refundable. No exceptions please. Students must be registered and have paid tuition before attending class. A registered student will be charged for their reserved place in a class until the end of the applicable term unless they turn in written notice of cancellation. Students will be charged for the month in which the cancellation occurs unless cancellation occurs before the first scheduled monthly meeting of that class.
Tuition is based on an annual schedule and calculated as four one hour classes per month for example if you are registering for a class that meets once a week, 8 one hour classes for that class that meets twice a week and so on. Some months have additional classes when there are five weeks in the month. You are not charged extra in the months that have additional classes but these classes are taken into consideration for holiday closures and used to insure the average classes per month for the annual session. In addition, during recital time extra rehearsals contribute to insure you receive the correct average number of classes and many times exceed the average calculated for
tuition. If you have any questions concerning this, please ask front desk for a reference of classes for the year.

Late Payments: a $15.00 late fee will be charged on all tuition paid after the 15th of each month. All Past due tuition balances that are not paid will be turned over to small claims court for collections. You will be charged court costs and legal fees should this become necessary.
⦁ Make-Up Lessons: The instructor or front desk will advise you as to the days and times of make-up classes (a students regular class time cannot be used as make-up class and the student must be actively enrolled in order to make-up missed classes.) Absences must be made-up within one month of the absence, otherwise they are forfeited. No tuition adjustments will be made for missed classes or forfeited make-­ up lessons. Additional rehearsals will count toward make-up classes.
Studio Hours: For help with any questions, Receptionists hours are 10:00am to 7:00pm, Monday throughFriday, and 9:00am to 4:00pm on
Saturday.

Open House: December is the month for our Studio Holiday Open House. Don’t miss this opportunity to see your child’s achievements. Your child’s invitation will be given out the first part of December and will provide you with dates and Open House class times.
Recital: Our annual recitals are held at the end of our spring session depending on facility availability. Please check at the front desk for exact date. There is a recital fee due· by the end of February. Costumes need to be sized and a deposit paid by March 1st. Additional detailed information about recital and costumes will be available in February. Information regarding pictures and videos will be made available in March. Recital is an exciting time and a memorable experience for your children and our staff. Please plan to participate.
Student Supervision: Children must be supervised and picked up immediately after class. Unaccompanied students should arrive no earlier than 15 minutes prior to their class. Our Receptionist cannot be responsible for students arriving earlier. Make sure your child has gone to the bathroom before classes begin. Please remind your child when waiting to be picked up that they are to remain in the building.
Visitors and Parents: All visitors and parents are asked not to enter the teaching studios. All parents and students who are waiting are asked to wait in the studio lobby area.
Dress: Please see the listing of class attire for each class. Street clothes or cover-ups should be worn over leotards and tights when arriving for class and when leaving the studio. Never wear your Ballet or Dance shoes from parking lot into building.
Dressing room facilities are limited, so we ask that you come dressed for class under appropriate street clothes or cover-ups. No street shoes on dance floors. We are not responsible for lost or missing articles – do not bring valuables.
Parking: When parking in our parking lot, please lock your car and do not leave valuables in plain sight in your car. We cannot be responsible for stolen items or vehicles. Please DO NOT park in the GECU Plaza parking lot next door, as the owner is towing all cars belonging to our patrons.
Holidays: Most public school holidays will be observed. Check our holiday Schedule and Bulletin Board for Studio Holidays.
Admission to class: Please be aware that no student will be allowed into class unless they are properly registered and tuition payments current.
Returned checks: There will be a $35.00 fee on all returned checks.

Class Attire

Below is a list of the required attire for each class.
Ballet Classes
Basic Leotard (NO Designs or Skirts attached)
*Pink Lycra Camisole leotards will be used for recital
Hair: Instructor requires that girl’s hair be worn up in a bun off face.
Pre- Ballet I-III: Pink leather ballet slippers, pink footed or convertible tights, pink leotard.
Basic I & II: Pink leather ballet slippers, pink footed or convertible tights, pink leotard and pink elastic waistband.

Technique I & II: Pink ballet slippers, pink footed or convertible tights, pink or black and a pink or black waistband.

Intermediate & Advanced: Pink ballet slippers, pink footed or convertible tights, any solid color leotard.

Adult Ballet: Pink ballet slippers, pink footed or convertible tights, any solid color leotard.

Pointe I & II: Pink pointe shoes, pink footed or convertible tights, any solid color leotard. (Ballet skirts, knit ballet shoes and leg warmers are optional)

Boys Ballet: White or black ballet slippers and male tights, shorts and white t shirt.

Men’s Ballet: White or black ballet slippers and male tights.

Pre-Jazz Classes: Leotard, dance shorts, shorts, skirt or jazz pants, suntan footed or convertible tights, black or tan jazz shoes.

Jazz I, II, III & IV Classes: Leotard, dance,shorts or jazz pants, suntan footed or convertible tights, black or tan jazz shoes, dance paws or Footundeez.

Lyrical/Contemporary: Leotard, dance,shorts or jazz pants, suntan footed or convertible tights, black or tan jazz shoes, dance paws or Footundeez.

Music Theater: T-shirt, black leggings or jazz pants, black or tan jazz shoes, black tap shoes, hair back and pulled away from face.

Hip Hop: Leotard, dance shorts or jazz pants and black sneakers.

Breakdancing: Knit Beanie

Folklorico: Black short sleeve leotard, red practice skirt, bloomers, bun cover, suntan tights, folklorico shoes. (Practice skirt and bloomers and
costumes may be ordered in Distractions)

Martial Arts: Black Kung Fu pant, Black Kung Fu Tee, Royal Gi Jacket.

Gymnastics: Black or white gymnastics shoes (gym shoes are required in gym room at all times for health and safety reasons) Absolutely No bare feet allowed in Gym room. Biketard (optional) No jewelry during gym/tumbling classes (earrings, rings; bracelets, piercings) for safety reasons.

Personal Items: We suggest that you come dressed with a cover-up. Dance shoes should NEVER be worn outside the building, they should be put on once inside the building prior to entering the classroom. Please remember to put all your clothes and shoes in your dance bags and do not leave in dressing rooms. Mark all clothes with your name. Champion Studio is not responsible for lost or stolen items.

Medical Release

We at Champion Studio seek to offer our customers the best studio instruction possible. Among our most important concern is professional instruction and safety. To achieve this, it is necessary that our instructors have experience, training, and a philosophy where health and safety. Naturally safety is of special importance. Champion must require the present release form to be signed by all families who have students participating in our programs.

While Champion does all in their power to ensure the safety of its students and to avoid conditions that could cause accident, it is not possible for Champion to guarantee absolutely safety. However, Champion in no way concludes from this that strict safety rules are not of the utmost importance. They absolutely are- at all times – in all places.
Champion only declares that not all accidents are foreseeable, even with the best safety precautions. Therefore, Champion has developed a release form which compiles ERISA requirements. All participants must read and sign this form in order to enroll in such classes. Thank you for your patience and understanding and may all students get the very most out of their classes!

PARTICIPANT AGREEMENT, RELEASE AND ACKNOWLEDGMENT OF RISK
I, the undersigned in consideration of the services and the reasonable rate provided for the same by Champion Studio, its officers, employees, agents or representatives (herein referred to as “Champion”), hereby agree to release and discharge Champion on behalf of myself, my heirs assigns, personal representation an estate, including a child or war for whom I sign, as follows:
I understand and acknowledge that the activity in which I, my child or ward are about to engage voluntarily as a participant and/or volunteer bears certain known risks and unanticipated risks that could result in injury, death, illness or disease, physical or mental, or damage to myself, my child or ward to my property or to spectators or other third parties. Participation in this activity is purely voluntary; no one is forcing me, my child or ward to participate, and I, my child or ward elect to participate in spite of its risks.
I understand that Champion does not wish to frighten me or reduce my enthusiasm for the activity, but believes it is important for me to know in advance what to expect and to be informed about the activity and its risks.
In gymnastics, the activities in which I or my child or ward for whom I sign will be involved include tumbling, flips in the air, exercises on uneven parallel bars (at least 4” and 7” from the ground), walking and cartwheels and other activites on a balance beam (at least 3 ½ from the ground), jumping from a springboard or mini-trampoline onto a pommel horse; and other gymnastics exercises and activities with and without a spotter or spotters, depending on the level of experience and expertise.
In all programs, the risks associated with the activities include those ordinarily associated with falling and colliding with others. Some, but not all of these risks are breaks, sprains, strains, bruises, cuts and abrasions, including the possibility of head injuries. Some of these risk and injuries may be serious. The description of these risks may results in injury, death or property damage. I agree to assume responsibility for the risks identified here and those risks not identified.
Being aware that this activity entails known and unknown risks or injury to myself, my child or ward and a risk of injury to spectators or other third parties as a result of my, my child or ward’s action, I expressly agree, covenant and promise to accept and assume all responsibly and risk of injury, death, illness or disease or damage to myself, my child or ward, to others or to my property or the property of others arising from my, my child or ward’s participation in this activity.
I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify Champion, it agents or employees, and all other persons or entities from any and all liability, claims, demands, actions and rights of action which are related to, arise out of, or are in any way connected with my, my child or ward’s participation in this activity, including specifically but not limited to the negligent acts or omissions of Champion, its agents or employees, and all other persons or entities, for any and all injury, death, illness or disease, and damage to myself, my child or ward or to my property. IN SIGNING THIS DOCUMENT, I FULLY RECOGNIZE THAT IF ANYONE IS JURT OR PROPERTY IS DAMAGED WHILE I , MY CHILD OR WARD IS ENGAGED IN THIS EVENT, I WILL HAVE NO RIGHT TO MAKE A CLAIM OR FILE A LAWSUIT AGAINST CHAMPIONOR IT OFFICERS, AGENTS OR EMPLOYEES ON BEHALF OF MYSELF, MY CHILD OR WARD EVEN IF THEY OR ANY OF THEM NEGLIGENTLY CAUSED THE BODILY INJURY OR PROPERTY DAMAGE. HOWERVER, I DO NOT WAIVE THIS RIGHT IN THE EVENT OF GROSS NEGLIEGENCE i.e. WILLFUL OR WANTON MISCONDUCT.
Should it become necessary for Champion, or someone on its behalf, to incur attorney’s fees and costs to enforce this agreement or any portion thereof, I agree to pay the reasonable costs and attorney’s fees thereby expended, or for which liability is incurred.

I certify that I have sufficient health, accident and liability insurance to cover any bodily injury or property damage, I my child or ward may incur while participating in this event and to cover bodily injury or property damage caused to a third party as a result of my, my child or ward’s participation in this event. If I have no such insurance, I certify that I am capable of personally paying for any and all such expenses and liability.

I, my child or ward have no past or present medical or psychological condition that might affect my, my child or ward’s participation in the activity or cause harm to myself, my child or ward or others.
My signature below reflects that I have read this entire document, understand it completely, understands that it affects my legal rights , and agree to be bound by it terms. References herein to “I” “my,” “myself,” and other first-person references shall include any child or ward for whom I sign.
